FIREHIRE® Inc. Liability Waiver
|
| If you have been requested by a staff member of FIREHIRE® Inc. or if would like to download a copy of the FIREHIRE® Inc. examination process liability waiver, please click here. This waiver must be on file at the offices of FIREHIRE® Inc. or you will not be able to participate in the examination process. These waiver are included in the above application packet, so if you have already downloaded an application packet, there is no requirement to download a waiver. If you are re-testing on your written examination to improve your score, you DO need to submit an updated waiver. |

Advanced Life Support Assessment Center Scoresheet
|

If you would like to download the Advanced Life Support Practical Assessment Score sheet, please click here. This score sheet conforms to the National Registry® Standards for a Cardiac Arrest (Mega-Code). You will be given a copy of this score sheet, after successful completion of the FIREHIRE® Inc. Physical Ability Test (FPAT). If you have recently completed Paramedic school and you have only completed the FIREHIRE® Inc. Basic Life Support Assessment center, it is your responsibility to contact FIREHIRE® Inc. to schedule your Advanced Life Support Assessment center. You will not be eligible for any ALS positions until you successfully pass the Advanced Life Support Assessment center.

FPAT (FIREHIRE® Inc. Physical Ability Test) Description

| If you would like to download a complete description of the FPAT (FIREHIRE® Inc. Physical Ability Test), please click here. The FPAT has a time limit of 15 Minutes. The FPAT consists of seven validated events that are based on an extensive firefighter job analysis. The events are based on fireground operations that a single-role firefighter might be expected to do during the course of an actual emergency. This is a pass/fail event. |

CPAT (Candidate Physical Ability Test) Description

If you would like to download a complete description of the CPAT (Candidate Physical Ability Test) guidelines, please click here. The CPAT has a time limit of 10 Minutes and 20 Seconds. FIREHIRE® Inc. DOES NOT administer the CPAT at this time. It is the candidates’ responsibility to obtain a CPAT completion certificate from any agency that administers the CPAT as part of their examination process, or a CPAT completion card issued by the CJAC committee before the cut-off date listed on the current FIREHIRE® Inc. job announcements. The CPAT completion certificate or completion card MUST NOT BE OVER ONE YEAR OLD from the date of the written examination. This Candidate Physical Ability Test (CPAT) consists of eight separate events. This test was developed to determine if candidates are physically able to perform essential job tasks at fire scenes.
